How @home helped Sally on her road to recovery
A new video from Lambeth Together’s Neighbourhood and Wellbeing Delivery Alliance featuring local resident Sally shines a spotlight on the difference receiving a hospital level of service at home can make. Sally recovered at home during a period of intense illness, where she would usually have required a hospital stay.
In the film, Sally shares her personal experience of the @home service, and reflects on how the support helped with her healing and recovery. Having benefited from the service herself, Sally is now a strong advocate, sharing her story to raise awareness of how multidisciplinary teams in Lambeth are helping more people receive expert care and recover at home.

About the @home service
The @home service featured in the film is a “hospital at home” service run by Guy’s and St Thomas’ NHS Foundation Trust for residents of Lambeth and Southwark. The @home team featured here works from their base at Elmcourt Health Centre. There is another service based at Walworth Road. Rather than being admitted to hospital, or staying in hospital for longer than clinically necessary, eligible patients can receive hospital-level care in their own homes from a specialist team.
Care is delivered by a multidisciplinary team that can include doctors, nurses, physiotherapists, occupational therapists, pharmacists, social workers and nursing rehabilitation support staff. Together, they provide personalised care for conditions such as infections, heart failure, exacerbation of chronic obstructive pulmonary disease (COPD), diabetes complications, falls, hyperemesis and end-of-life care, bringing expert treatment and support directly to patients at home.
