@Home services provide hospital-level care to patients in the comfort of their own homes for conditions that may traditionally have required admission or hospital-based management. By delivering safe, coordinated care in the community, the service helps to prevent unnecessary hospital attendance and admissions, supports earlier discharge, and improves patient experience.
The model promotes independence, recovery in familiar surroundings, and more personalised care, while reducing pressure on acute services and strengthening community-based provision.
By bringing together specialist healthcare professionals from across the NHS and community services, @Home promotes independence, improves patient experience and strengthens neighbourhood-based care while reducing pressure on hospital services.

How @Home works

The @Home service is delivered by a multidisciplinary team based at Elmcourt Health Centre, providing care for eligible residents across Lambeth and Southwark.
Rather than remaining in hospital, or being admitted in the first place, eligible patients can receive expert treatment at home from a team that may include:
• Doctors
• Nurses
• Physiotherapists
• Occupational therapists
• Pharmacists
• Social workers
• Rehabilitation support workers
The service supports people with a range of conditions, including infections, heart failure, COPD, diabetes complications, falls, dehydration and end-of-life care, ensuring they receive coordinated, high-quality care in the place they are most comfortable.

Sally’s story

A new film from Lambeth Together tells the story of local resident Sally, who received care from the @Home service during a period of serious illness.
Instead of staying in hospital, Sally was able to recover in her own home with support from the multidisciplinary @Home team. In the film, she reflects on how receiving care in familiar surroundings helped her recovery and provided reassurance to both herself and her family.
Having experienced the service first-hand, Sally is now a passionate advocate, helping raise awareness of how neighbourhood teams are supporting more residents to receive hospital-level care at home.
