We’re bringing services together based on people’s needs and where they live. We’re doing this through three ‘Delivery Alliances’.
The Living Well Network is making it easier for people to get help with their mental health needs. It already supports hundreds of people every month.
This Alliance will bring together different providers to offer a better service to Lambeth residents facing a range of issues, including long-term illnesses.
The Children and Young People Alliance will work to make Lambeth one of the best places in the world for children and young people to grow up.

Over the past eight months, a series of inspiring cancer awareness events have been making a real impact in tackling health inequalities across Lambeth. The events were organised by social prescribing link workers from Lambeth’s Primary Care Networks, and with thanks to funding from Lambeth Together’s Neighbourhood and Wellbeing Delivery Alliance and the South East London Cancer Alliance.
HEAL-D is a seven week diet and lifestyle programme open to Lambeth residents. It has been culturally tailored and developed to support and empower people of African and Caribbean heritage, who are living with type 2 diabetes, to take charge of their health whilst enjoying the food from their culture.
Lambeth Together is proud to present its annual review, showcasing significant progress in health and care plans for the borough. Their commitment to improving health and wellbeing, delivering high-quality care, and creating a sustainable health and care system for Lambeth communities remains stronger than ever.
The transition from paediatric to adult care is often described by young people and their families as one of the most stressful and under-supported phases in healthcare. Yet, there is strong evidence that effective transitional care programmes can significantly improve health outcomes.
