Lambeth partnership to coordinate joined-up health and care through new integrator role
A new partnership between Guy’s & St Thomas’ NHS Foundation Trust (GSTT) and the Lambeth General Practice Provider Alliance has been confirmed to take on the role of Neighbourhood Health Service Integrator for Lambeth.
Approved by the South East London Integrated Care Board (ICB) on 16 July, this partnership will help coordinate and connect health and care services across the borough—supporting more joined-up, community-based care that meets the needs of local people.
Andrew Eyres, Corporate Director of Integrated Health and Care, Lambeth, NHS South East London and Lambeth Council said: “This is about making care work better for people in Lambeth. By bringing together GPs, hospitals, social care, and community services, we can help ensure people get the right support, in the right place, at the right time. This is a real opportunity to build on the strengths of our local partnerships.”
What is a Neighbourhood Health Service Integrator?
The integrator is not a new service or organisation, but a coordinating role that helps different parts of the health and care system work together more effectively. It provides the infrastructure, relationships, and support needed to build integrated neighbourhood teams—local teams of professionals from across health, social care, and the voluntary sector.
These teams will focus on:
- Supporting people living with frailty
- Managing multiple long-term conditions
- Coordinating care for children with complex needs
The partnership will work closely with Lambeth Together’s Neighbourhood and Wellbeing Delivery Alliance to make sure local insight and population health data shape neighbourhood-level solutions and reflect the needs of Lambeth’s diverse communities.
