Help shape health and care services in your neighbourhood!

Healthwatch Lambeth is inviting people who live in Lambeth, use a Lambeth-based GP surgery, or care for someone in the borough to share their views on how local health and care services should evolve.

As part of Lambeth Together’s commitment to more joined-up, community-based care, your feedback will help shape the development of Integrated Neighbourhood Teams (INTs) — a key priority in the 10 Year Health Plan for England.

INTs bring together professionals from health, social care and voluntary services to work as one team supporting people in their local area. These teams aim to help residents stay well, live independently, and avoid unnecessary hospital visits.

At first these neighbourhood teams will focus on:

  • Children and young people with complex needs: for example, people with long-term health-conditions, mental or emotional needs, neurodiversity, behavioural needs and social or economic pressures within their family.
  • People who have multiple long-term health conditions: such as diabetes, high blood pressure or heart disease.
  • People who are frail: this may include people with a slow walking pace, someone who’s having difficulty recovering from illness, low energy or weakness, or a higher risk of falls. And other challenges which may lead to hospital admission. Frailty isn’t just a concern for older people, it can happen at any age.
  • People in need of mental health support: for example people with depression, anxiety, and trauma.

If you live in Lambeth, your GP is based in Lambeth, or you care for someone who lives in Lambeth – your views matter and are important. Responses are especially welcome from people with lived experience in one or more of the areas highlighted above.
