On 7 and 8 June 2025, Lambeth Together brought health and wellbeing services directly to the heart of the community at the Lambeth Country Show in Brockwell Park. As part of this much-loved annual event, the team engaged with local residents, offering free health checks, expert advice, and practical resources to support healthier living.
The transition from paediatric to adult care is often described by young people and their families as one of the most stressful and under-supported phases in healthcare. Yet, there is strong evidence that effective transitional care programmes can significantly improve health outcomes.
