Lambeth Together celebrates progress and community engagement

Lambeth Together is proud to present its annual review, showcasing significant progress in health and care plans for the borough. Their commitment to improving health and wellbeing, delivering high-quality care, and creating a sustainable health and care system for Lambeth communities remains stronger than ever. And residents are reminded they are always welcome to take part in shaping how local health and care services are run.

Whether it’s sharing your views on health and care, attending the public forum, or speaking to the leadership team at the Lambeth Country Show, there are many ways to influence the work of Lambeth Together.

Cllr Nanda Manley-Browne, Lambeth Together Care Partnership Board Co-Chair and Cabinet Member for Healthier Communities (job-share) said: “Our achievements this year are the result of deep and collaborative partnerships across the borough. Together, we’ve been able to prioritise prevention, tackle longstanding inequalities, and focus on the wider factors that shape our health.

“As a borough committed to equity and justice, it’s vital that our communities feel heard and involved in the decisions that affect their lives. Whether it’s through our public forums, community outreach, or events like the Lambeth Country Show, we want to hear from you. Tell us what’s working — and what still needs to change — so we can continue shaping services that work for all our residents.”

Highlights from the annual review

Some key achievements in 2024 included:

  • Improving health and wellbeing in Lambeth: the joint Carers’ Strategy, All-Age Autism Strategy, and Age Friendly Action Plan have been pivotal in addressing the wider determinants of health. Blood pressure management among Black and multi-ethnic residents has improved, with a higher proportion achieving good control rates.
  • Delivering high-quality care for residents: Lambeth Together’s strength in partnership has improved access to health and care in the community. The recent Department for Health and Social Care Health Checks at Work pilot programme provided cardiovascular checks to over 4,000 people. General practice increased the number of appointments, NHS App registrations, and patient satisfaction, while improving ease of access for patients. The Lambeth Together Health and Wellbeing Bus supported over 1,000 residents and workers each month. And the Substance Misuse Programme achieved significant increases in treatment numbers, co-produced a new young person’s drug and alcohol service, and established the first of it’s kind Chemsex Forum.
  • Strengthening community hospital services: The Hospital @home service has expanded with improved links with palliative care and respiratory teams, and a new pilot project for sickle cell patients. A Carers’ Hospital Discharge pilot also supported carers during hospital discharge by linking them with information, support, and post-discharge interventions.

  • Get involved with open engagement projects: participate in open surveys or other opportunities to share your views with organisations across the partnership. Get involved.
