The Neighbourhood & Wellbeing Delivery Alliance urgent and emergency care priority outcomes

Neighbourhood & Wellbeing Delivery Alliance (NWDA) is acting to achieve the following as a result of our collaboration with local partners:

  • Ensure Lambeth people know where to go to get the right help, and are treated at the right time, in the right place, by the right person for their needs
    • Ensure people can access the right support in the right place at the right time, utilising the most appropriate help including primary care, community pharmacy, 111, urgent treatment centres and emergency departments
    • Increase the number of people attending hospitals that are treated and go home on the ‘same day’
    • Ensure people admitted to hospital are discharged in an appropriate timeframe with a reduction in preventable delays
    • Ensure people needing scheduled treatments are suitably prioritised and any unnecessary waits are reduced
  • Ensure Lambeth people have access to joined up and holistic health and care throughout integrated neighbourhood teams
    • Ensure people are supported by genuinely integrated teams of GPs, community health and social care staff
    • Provide virtual wards to allow patients to get the care they need at home safely and conveniently, rather than being in hospital
